On Wednesday, the U. S. Food and Drug Administration said it will begin testing Xenical (orlistat) on more than 1,000 people who are taking it for an inflammatory bowel condition. FDA spokeswoman Emily Carter said that is the largest-ever Xenical test ever. "We've just received a very strong response from the FDA from the FDA to test this drug in people with inflammatory bowel disease," said Carter, a clinical investigator at Vanderbilt University.
Because there is not enough information to make a conclusion on whether the drug will work for these people, the FDA is now deciding whether to start the test after it has already been approved by the FDA.
"It's an important drug," said Dr. Thomas W. Weisman, a clinical pharmacology professor at Vanderbilt University. "We haven't been able to test for people with inflammatory bowel disease who are not on the medication."
The FDA is not saying it will stop using the drug before the end of the year. But the agency hopes to make it available sooner. "We've had some good feedback from the public and from patients that the drug is safe and effective and has been shown to reduce the risk of death from bowel cancer," said Dr. David W. Rosenblum, a clinical assistant professor of pharmacology at the University of California, San Francisco. "But now we have a better idea."
In June, the FDA will begin testing a drug called Xenical, the same drug Xenical has been used to treat patients with inflammatory bowel disease since 1998.
Xenical is the active ingredient in the prescription drug Orlistat, a medicine taken for the treatment of obesity, or a medical condition that prevents the body from absorbing calories from food. The drug is available by prescription only. The Food and Drug Administration will require a patient's doctor to write a prescription for Xenical or another weight loss treatment, but there is no information that the drug is safe for people with inflammatory bowel disease.
"If the drug is not approved by the FDA, the FDA will have the drug withdrawn from the market," said Dr. Mark W. McKillop, a director of the Center for Drug Evaluation and Research.
Xenical was approved by the FDA in 1999 for the treatment of obesity. Since then, the drug has been used to treat bowel disease that occurs in the presence of inflammation. The Food and Drug Administration says it was approved for use by millions of people in 1998, but only a minority of the people in the study were taking the drug.
The drug has been available to millions of people for more than 40 years and has been shown to reduce the risk of gastrointestinal and cardiovascular diseases. The drug has also been shown to reduce the risk of Alzheimer's disease, diabetes and other diseases that cause problems with the nervous system.

Xenical is being tested in people with inflammatory bowel disease that occur in the presence of other medical conditions, such as Crohn's disease.
The drug was originally tested in rats, but was found to have no effect on the gastrointestinal or cardiovascular system. It was tested in humans only. "It should not be used as a treatment for bowel disease," Dr. K. Dukis said.
The drug should not be used in people who have a history of gastrointestinal problems such as ulcerative colitis, Crohn's disease or other bowel disease.
